This position reports to the Program Manager of Homeless Health Services and functions as an essential member of Honor Community Health's Homeless Health Services team. The Community Health Worker I - Homeless Health Services role is an essential member of Honor Community Health's Homeless Health Services team and serves as a liaison, advocate, and resource navigator for individuals experiencing homelessness, housing instability, and other complex health and social needs.
The Community Health Worker (CHW) works collaboratively with an interdisciplinary team to improve access to medical care, behavioral health services, substance use treatment, housing, public benefits, and community resources. Services are provided in health centers and community-based settings, including shelters, transitional housing programs, hospitals, community partner locations, encampments, and other approved locations.
The CHW uses trauma-informed, person-centered, recovery-oriented, and culturally responsive approaches to engage patients, identify barriers to care, support patient-identified goals, and help patients successfully navigate healthcare and community service systems.
What are we looking for?
- A High school diploma or equivalent is required.
- One (1) year of experience working in healthcare, behavioral health, substance use treatment, homeless services, human services, community outreach, or a related setting preferred, including experience working with vulnerable populations with a knowledge of community resources, housing systems, public benefits, behavioral health services, and substance use treatment resources.
- Current MiCHWA Community Health Worker certification or the ability to obtain within 12 months of hire and maintain certification throughout employment.
- Must obtain and maintain all credentials and enrollment required for Medicaid reimbursement of Community Health Worker services, including MI Medicaid CHW Registry and CHAMPS enrollment, as applicable.
- Strong computer skills Epic and Microsoft.
- Valid Michigan driver's license and reliable transportation required.
