Position Description
Reporting to the Chief of Behavioral Health, the Targeted Case Manager performs interventions to support the wellness and recovery goals of individuals with complex and/or chronic behavioral health issues across the continuum of care. This includes implementing targeted interventions to provide timely, high-quality, and efficient care. The role focuses on providing access to services that help individuals resolve problems interfering with their independence or self-sufficiency and maintain themselves in the community.

Our standard hours are Mon-Fri, 8:30am-5:00pm.

What are we looking for?

A bachelor's degree in social work, psychology, nursing, rehabilitation, education, occupational therapy, counseling, or community mental health.
CCM certification
CADC/CAADC preferred
Experience in human services, social work, and/or public health preferred.
At least one year of experience in providing direct services to mental health and/or substance use disorder consumers.
Experience working for a community health center or Federally Qualified Health Center preferred.
Spanish-speaking preferred.
